New Patient Care Advocate Joins Our Washington Clinics

 

One Community Health (OCH) welcomes Juana Bruno as our newest patient care advocate (PCA). Juana will be working at our Washington clinics. She is available to all community members whether you’re an OCH patient or not to help with Medicaid and Medicare related services and questions.

What is a patient care advocate?

A trained and certified team member that is experienced with Medicaid and Medicare services. They may also help with financial resources and social services.

As a PCA Juana can help with:

  • Washington Apple Health enrollment

  • Medicare resources including extra help for Part D

  • Low-income home energy assistance

  • Medicare savings programs to help cover premiums

Juana can help answer your questions or guide you through the application process

You may qualify for Apple Health if:

  • You are a Washington resident 19 to 64 years old

  • You are a U. S. citizen or meet Medicaid immigration requirements

  • You are not pregnant and haven’t had a pregnancy end in the last 12 months

  • You are not eligible for health plans with tax credits or federally funded programs

  • Your home meets certain monthly income requirements

Apple Health provides a broad range of coverage, including:

  • Physical and behavioral health services

  • Dental services

  • Transportation to Apple Health covered appointments

  • Interpreter services

  • Pharmacy services (over-the-counter and prescription drugs)

  • Vision hardware (for clients under age 21)

What to Bring to Your Appointment:

  • One month’s worth of pay stubs (if available)

  • Recent tax returns (if available)

  • Information on hourly pay rate and hours worked per day/week

  • Names and dates of birth for everyone in your tax household

If you don’t have all these documents available we can submit them after applying.
