New Patient?

Here at OCH, you have a team of people available to support you and your healthcare needs. Your care team includes your provider, nurses, medical assistants, referral coordinators, community health workers, and behavioral health consultants. They all work together to meet your needs and are available in-person, by phone, or through our patient portal, MyChart.

Preparing for your first visit

Schedule your appointment

Call or Text 541-386-6380 for Oregon locations, 509-493-2133 for White Salmon, and 509-427-4212 for Stevenson. We know that things come up and sometimes you need to cancel or reschedule your visit. Please call or text us at 541-386-6380 as soon as you know and at least 24 hours before your visit, so we can give your visit to another person waiting for the chance to visit with your doctor.

Paying for your bill and financial help

Our Financial Assistance Center includes more information on the insurance we accept and ways to pay your bill.  We encourage anyone who is uninsured to apply for Medicaid health insurance. Get help applying for Medicaid or apply for our Sliding Scale Discount Program.

Bring your medication

One of the first questions your new provider will have is about what medications you take.  Consider bringing them all with you to make it easy!

Bring your health history

If you have medical records from other clinics, we can help collect them from other providers.  If you haven’t started, you can fill out a request for medical records here.

Patient rights and responsibilities

One Community Health (OCH) seeks to maintain a respectful and caring environment for all patients. The goal of these Rights and Responsibilities is to encourage a positive relationship between you, your family, and your One Community Health care team. Your health and wellness are important to us.
